Arrive at the time of Check In (15 minutes prior to your scheduled appointment time) with:
- Your photo ID
- Your Insurance card
- A method of payment for copays, deductibles and/or supplements you may wish to purchase.

What to Expect at the visit?

During your first appointment a thorough health history will be taken and an appropriate physical exam and pertinent labs may also be performed. Bringing in any health information such as medical records and a complete list of your medications and vitamins/herbs is very helpful.

Parking:

We have plenty of parking on the level of the clinic which is accessible via Hemlock Way just East of 5th Ave S. Please park in the designated NSNM parking only spots. The spots down at street level just off 5th Ave S belong to the dentists downstairs and we want to be good neighbors and respect their spots.

Children Policy:

We do not advise bringing minor children to your appointment, but understand this may be sometimes be necessary.

Minors under 14 years of age must accompany their guardian during the appointment. Please only bring them if you feel confident they will not disrupt or distract from your time with the doctor.

Fragrance and Perfume Policy:

North Seattle Natural Medicine we strive to have a fragrance-free facility due to potential allergies of other patients and doctors. Please avoid wearing perfume/cologne/strong scents to keep our facility free from allergens.

Financial and missed/cancelled appointment policy:

We request that you give our office 24 hours notice in the event that you need to reschedule your appointment. If you arrive at the time of your new patient appointment, and not 15 minutes prior, we will likely need to reschedule you, especially if you do not have your intake forms completed. Our late appointment/cancellation fee is $50.00. All fees must be paid before or at the next visit. Please note, multiple incidents may result in termination of care at NSNM
